How can we help you?
We are a team of NHS healthcare professionals, travelling around Hounslow to deliver health checks – on the high street, in your community centre, in your place of work, or in your place of worship.
Access Criteria
We can offer information, guidance and blood pressure checks to all. Click on one of the tabs below to learn more about the Health Checks services we offer.
All who live, work, study and/or are registered with a Hounslow GP. Consent is required for us to take and record the Blood Pressure check.
Eligible residents are aged 25 to 74 years old who have not had an NHS Health Check in the last 5 years. Residents who have certain pre-existing conditions are also excluded, this would need to be discussed with our health professionals on the day. Our service is unique in targeting younger residents, the standard NHS Health Check available at any GP practice is only for those over 40 years old.
Eligible residents are those who are at higher risk of cardiovascular disease, in other words: smokers, no physical activity, elevated BMI or Q-risk greater than 10%. Eligibility is best determined through an NHS Health Check (see above).
Each service is targeted and tailored to a specific cohort of residents who will benefit the most. Eligibility can be discussed with our team on the day, although this is ultimately determined by the provider when they receive a referral.
Eligibility criteria for a comprehensive health check
Residents aged 25 to 74 years, who have NOT had an NHS Health Check in the last 5 years.
Residents who DO NOT HAVE any of the following health conditions:
- Heart Disease
- Diabetes
- Stroke
- Heart Failure
- Chronic Kidney Disease
- High Blood Pressure (Hypertension)
- Inherited high Cholesterol (Familial Hypercholesterolemia)
- Atrial Fibrilation
- Transient Ischemic Attack
- Peripheral Arterial Disease
- Currently being prescribed Statins to lower Cholesterol
- Previous checks have found that you have a 20% or higher risk of getting Cardiovascular Disease over the next 10 years
