How can we help you?

We are a team of NHS healthcare professionals, travelling around Hounslow to deliver health checks – on the high street, in your community centre, in your place of work, or in your place of worship.

About our service

We usually operate out of our Healthy Hounslow ambulance, which has all the equipment we need to deliver our full range of health check services! We also attend some community events without the ambulance where we can deliver some (but not all) health check services.

This is a walk-in service and we operate on a ‘first come first served’ basis. Unfortunately, we do not accept referrals. If you would like to know about upcoming events or the location and timings of our ambulance clinics, please have a look at the events calendar. We look forward to seeing you!

WHAT? 

We offer a Blood Pressure check and record your reading on your NHS patient record so your GP or any other health professional can follow up with you if needed. Our knowledgeable staff are here to answer any questions you may have on your Blood Pressure reading, healthy lifestyles, cardiovascular risk, and what that means for you.

WHO? 

All who live, work, study and/or are registered with a Hounslow GP. Consent is required for us to take and record the Blood Pressure check.

WHAT? 

Our converted ambulance offers efficient NHS Health Checks, a leading prevention program with 1.5 million annual check-ups. This 30-minute assessment gauges the risk of health issues like diabetes and heart disease. We then check your blood pressure and do a blood test. Residents receive a detailed discussion of results, empowering them to manage their cardiovascular risk.

WHO?

Eligible residents are aged 25 to 74 years old who have not had an NHS Health Check in the last 5 years. Residents who have certain pre-existing conditions are also excluded, this would need to be discussed with our health professionals on the day. Our service is unique in targeting younger residents, the standard NHS Health Check available at any GP practice is only for those over 40 years old.

WHAT?

We have Health & Wellbeing coaches on hand to talk with residents about managing their cardiovascular risk, supporting and motivating them to make changes to their lifestyle. If interested and eligible, residents would be able to access up to 6 coaching sessions over a 12-week period.

WHO?

Eligible residents are those who are at higher risk of cardiovascular disease, in other words: smokers, no physical activity, elevated BMI or Q-risk greater than 10%. Eligibility is best determined through an NHS Health Check (see above).

WHAT?

We are partnered with Morelife, Lampton Leisure and Beezee to provide community physical activity on referral, weight management, healthy eating, smoking cessation, additional health coaching, cook-and-eat sessions and other learning opportunities. On the day, we would be able to refer eligible residents to a range of lifestyle interventions – encouraging everyone to move more, eat well, stop smoking, drink less, sleep well and stress less.

WHO?

Each service is targeted and tailored to a specific cohort of residents who will benefit the most. Eligibility can be discussed with our team on the day, although this is ultimately determined by the provider when they receive a referral.

About the PCN

The CVD Outreach service is delivered in partnership with Feltham & Bedfont Primary Care Network and Hounslow and Richmond Community Healthcare NHS Trust.

Feltham & Bedfont Primary Care Network is an ambitious network of GP practices, developing at pace. As well as delivering health services commissioned by NHS England and North West London, Feltham & Bedfont PCN has been working hard to address health inequalities and deliver health outreach services to residents.

Hounslow and Richmond Community Healthcare NHS Trust offers over 70 health services for 523,000 residents in Hounslow and Richmond, extending to south London for specialised services like Health and Wellbeing, school-aged immunisations, and newborn hearing screening. With around 1,300 employees, HRCH had 974,627 patient contacts last year, primarily focusing on community healthcare in homes, clinics, and children’s centres.
